Patients will receive an individual invitation code to create a secure communication portal with their healthcare provider
Healthcare providers can customize and personalize questionnaires. Each questionnaire can be tailored directly to information needed from an individual patient
Questionnaire data is sent securely to the patient and date is securely sent back to the healthcare provider
Patients will receive a notification once a new questionnaire is available
Patients will have a history of questionnaire responses available at the tap of their fingertip
